Hvis tungebåndet er kort, så kan tungen hjælpes ved at træne specifikke bevægelser. Når tungen har lært disse bevægelser, så kan det vurderes, om det er nødvendigt at klippe tungebåndet. Efter et klip skal tungen igen træne andre specifikke bevægelser.

På kurset lærer du at vurdere, hvordan tungebåndet påvirker tungens bevægelser. Det handler ikke kun om, hvordan tungebåndet ser ud. Derfor foregår undersøgelsen som analyse af tungens bevægelser og man lærer konkrete øvelser som skal foretages før klip af stramt tungebånd, og øvelser som skal foretages efter klip af tungebånd. Det er vigtigt at lave specifikke øvelser før et klip, så der er sensorisk tolerance for berøring i munden og så den motoriske planlægning af tungens bevægelser er indøvet, for så har øvelserne efter et klip større effekt.  

Kurset omhandler, hvilken indflydelse stramt tungebånd har på:

  • spisning
  • synkning
  • amning
  • tale

Du lærer at foretage funktionel undersøgelse af tungebånd i relation til spise og tale.

 

Kurset foregår på letforståeligt engelsk. Lori er vant til at undervise ikke-engelsktalende terapeuter i hele verden.

Underviser: LORI OVERLAND, MS, CCC-SLP, C/NDT

Lori Overland is a speech and language pathologist with more than 35 years of professional experience. Lori specializes in dealing with the unique needs of infants, toddlers, pre-schoolers and school-aged children with oral sensory-motor, feeding and oral placement/speech disorders. She has received an award from the Connecticut Down Syndrome Association for her work within this population. Lori consults with children from all over the world, providing evaluations, re-evaluations, program plans and week-long therapy programs. Lori also provides consults to local school districts and Birth-to-Three organizations. Her goal in addressing feeding and speech challenges is to improve the quality of life for both the children she serves and their families. In addition to her private practice, Alphabet Soup, Lori is a member of the TalkTools® speakers bureau. Lori has lectured on sensory-motor feeding disorders across the United States and internationally. Her classes, "Feeding Therapy: A Sensory-Motor Approach" and "Developing Oral-Motor Feeding Skills in the Down Syndrome Population" are approved for ASHA and AOTA CEUs. Lori is the co-author of A Sensory Motor Approach to Feeding. She holds degrees from Horfstra University and Adelphi University and has her neurodevelopmental certification.

Functional Assessment and Remediation of Tethered Oral Tissues (TOTs)
På kurset lærer man at foretage en praktisk og funktionel undersøgelse af tungebånd. Dvs. man lærer ikke kun at se, hvordan tungebåndet ser ud, men man hvordan tungebåndet påvirker tungens bevægelser, og hvordan tungetræning kan foregå. Lori Overland har sammen med Robyn Merkel-Walsh udviklet the Merkel-Walsh & Overland TOTs Protocol, som undersøger tungebåndets struktur, oralmotorisk funktion, før-spisefærdigheder, spisning og tale. Kursusdeltagerne lærer at vurdere, hvordan tungebåndet påvirker før-spisefærdigheder, spisning og udtale.
Undersøgelsen foregår som analyse af spisning og tale, og man lærer konkrete øvelser som foretages, før klip, og øvelser som foretages efter klip af tungebånd. Det er vigtigt at lave specifikke motoriske øvelser før et klip, så der er sensorisk tolerance for berøring i munden og så den motoriske planlægning af tungens bevægelser er indøvet, så øvelserne efter et klip får større effekt.
Deltagerne lærer at vurdere, hvordan stramt tungebånd påvirker forudsætningerne for amning, drikke af sutteflaske, drikke af kop, drikke af sugerør og spise fast føde samt tale. Der anvendes video og øvelser, så man kan implementere den nye viden straks efter kurset. Der diskuteres terapeutisk indsats før og efter klip af tungebånd og kirurgiske teknikker vises, så fagpersoner kan få et godt indblik i den aktuelle tværfaglige internationale viden på området.     
Course Description

Merkel-Walsh and Overland not only cover structure but the most important piece which is functional assessment of pre-feeding, feeding and speech that can be implemented in children through adults. Those results are then task analyzed to create a proper pre-and post-operative protocol.

 

Tethered Oral Tissue(s) or TOTs is a topic that has been more frequently discussed in the fields of lactation, speech pathology, oral surgery, orofacial myology and otolaryngology. Organizations such as the International Association of Orofacial Myology (IAOM) and the Academy of Orofacial Myofunctional Therapy (AOMT) have been highlighting this diagnosis and implications for therapy and surgery. There are three forms of TOTs: buccal, labial and lingual. Lingual TOTs is most commonly known as “tongue-tie” or “ankyloglossia”. The Mayo clinic in 2016 indicated that tongue-tie can affect the way a child eats, speaks and swallows, and can interfere with breast-feeding.

One of the driving factors in the increased interest in TOTs stems from current research linking sleep apnea to a short lingual frenulum, in addition to the increase awareness of the importance of breast feeding. With this topic comes some controversy, and speech pathologists are often left confused in their own role of diagnosing and treating TOTs.

This course explores the functional assessment of TOTs and reviews current well-respected assessment tools which describe structure. With these tools, Lori and Robyn have developed the Merkel-Walsh & Overland TOTs Protocol which looks at structure, oral motor function, pre-feeding, feeding and speech. Participants will learn to use this tool to identify the possible need for a TOTs referral, and the functional impact this diagnosis may have on feeding and speech. Their unique style of “task analysis” is a practical strategy for assessment that is immediately carried over into developing a treatment program for neuro-muscular re-education. 

Participants will learn through the concept of task analysis the specific ways in which TOTs impacts the oral motor and oral placement skills for breast, bottle, cup, straw and solid feedings in addition to speech sound production. Videos and live practicums with attendee participation will provide therapeutic intervention tasks that participants can implement with their clients immediately. The importance of pre and post-surgical therapy will be discussed, and surgical techniques will be explored so that therapists may be have a better understanding of the current research across disciplines on an international level.

Learning Outcomes

Participants will be able to:  

  1. list three forms of TOTs.
  2. identify at least 3-5 functional impacts of TOTs on feeding skills.
  3. list at least 3 speech sounds that may be affected by TOTS.
  4. list at least 2 professionals that should be consulted if TOTs is suspected after a functional assessment.
